A successful sterilization cycle does not guarantee that an item will remain protected.
The package is part of the sterile barrier. Once sterilized, it must remain intact through cooling, storage, transport, and distribution. If that barrier is compromised, sterility can no longer be assumed. Package inspection is a quality checkpoint, not just a final visual check.
How package integrity gets lost
A package can be compromised by:
- Moisture
- Tears or punctures
- Damaged seals
- Crushed corners
- Excessive handling
- Improper storage
- Transport damage
Even a small defect can affect the sterile barrier.
What should technicians check?
Before an item moves into storage or distribution, check for wetness, tears, punctures, damaged or incomplete seals, crushed packaging, and missing or incorrect labels.
If the package is damaged, don't treat it as cosmetic. Follow facility policy and applicable guidance for reprocessing. The CDC recommends checking sterilized packages for compromised integrity, including wetness, tears, and punctures.
A CDC reminder
The sterilization cycle is only one part of protecting a sterile product. The CDC explains that packaging must provide an effective barrier against microbial contamination and maintain the sterility of the processed item after sterilization. A sterile package should not be used if it is wet, torn, or punctured. Moisture is especially important because sterile items that become wet are considered contaminated.
The takeaway is simple: protecting sterility does not stop when the cycle ends. Inspect the package. Protect it during storage and handling. If the barrier is compromised, follow your facility's process for addressing the item.
A leadership responsibility
Package integrity should be a defined quality checkpoint, especially when departments are busy. Leaders should provide clear inspection expectations, minimize unnecessary handling, review storage and transport practices, and address repeated patterns of package damage.
If the same damage keeps happening, ask why — not just how often items need reprocessing. Is package integrity part of your department's quality checks?
